Simi Valley Girls Softball

In an effort to keep our girls safe, ASA and SVGS will once again be conducting background checks. Any manager, coach, team mom or any volunteer working on the field or in the dugout with the players or will be handling funds on behalf of the team, MUST complete an ASA Background check. The form has changed from last season and only this new form will be accepted. The good news is you don't have to provide your social security number unless requested at a future date. SVGS has also changed the process for completing your background check. Please follow the below process to complete your background check:

PROCESS:

• Download the Background Check form which can be found here
• Complete both pages of the Background Check form
• Attach a copy of your drivers license
• Attach a check payable to SVGS for $13
• Mail all of the above items to the SVGS P.O. Box

HERE'S THE NO'S:
No, your police background check is not acceptable - No, last year's check is not acceptable and No, any other check is not acceptable. You MUST go through the ASA background check annually.
